Description:
On 26. 7. 2025, the pilot of the Skylane v.5 ultralight aircraft was performing a series of VFR flights from/to LKNM with other persons on board. After landing from the last flight, the pilot left the runway and stopped near the TWR station. Another person on board unfasted himself, opened the door on the right side of the cockpit, got out and, while moving forward, came into contact with the spinning propeller at idle engine speed. The propeller hit the injured person in the head and left hand. The persons on the scene, together with the attending doctor, provided first aid to the injured person and called the emergency medical service and the Police of the Czech Republic. The injured person was transported by an HEMS helicopter to the hospital with a serious head and left hand injury, where she died from her injuries.
The ULL traffic inspector of the LAA CR arrived at LKNM and, in cooperation with the Police of the Czech Republic, began an on-site investigation. In accordance with Article 2(1) of Regulation (EU) No 996/2010, the event was classified as a serious incident, taking into account the circumstances of its occurrence.
The cause of the serious incident was the collision of the injured person with the blades of a rotating propeller as a result of spontaneous action or deliberate disregard for the pilot's instructions. The death of the injured person occurred solely as a result of an accident, as a result of injuries sustained in a collision with the blades of a rotating propeller.
The AAII issues the following safety recommendations:

The AAII recommends that The Light Aircraft Association of the Czech Republic supplement the relevant guidelines on safety briefing requirements and principles for instructing persons with regard to the risks that arise when flying with another person on board of sports flying devices.
